https://www.211unitedway.org/ – United Way 2-1-1 is an easy-to-remember, three-digit number that families and individuals in Minnesota can call to obtain free and confidential information on health and human services. 2-1-1 call centers are staffed 24/7 by trained Community Resource Specialists who quickly assess needs and refer callers to the help that they seek.

Long-term care 


https://mn.gov/ooltc/ -The Office of Ombudsman for Long-Term Care (OOLTC) advocates for Minnesota adults needing or receiving long-term care.

Mental Health

MnCHOICES is the name of Minnesota’s assessment and support planning tool used by counties, tribal nations and managed care organizations.Certified assessors and care coordinators work with people of any age with a disability or anyone in need of long-term services and supports. They use MnCHOICES to discuss your preferences and the supports you need to live a good life. Your responses determine eligibility for a number of public programs including home and community-based services waivers and personal care assistance. Its rules-based system ensures that everyone is treated fairly, no matter where they live.


Crisis Management

Hennepin County 


Call **CRISIS (274747) from anywhere in the State of Minnesota to reach the local County crisis team. If you’re in a mental health crisis or know someone who is, we can help.

The Cope mobile crisis teams can come to where you are. The teams respond to anyone in the county who needs an urgent response.

Ramsey County

 Anoka County


24-7 Mobile Crisis Services at 763-755-3801.

Call 911 if you or others are in immediate physical danger and need help from the police, fire department, or an ambulance. 

Call 988 if you or someone close to you is having a mental health crisis. The Anoka County Mobile Crisis Response team will help you. The line is open all day, every day, and the call is free. The Crisis Response line is for both adults and children.

Text MN to 741741 to be connected with a counselor who will help defuse the crisis and connect the texter to local resources. Crisis Text Line is available 24 hours a day, seven days a week.
